The imaging findings can be non-specific initially, but there are some specific radiologic clues that can aid diagnosis. Because early treatment with antifungal agents improves survival, the radiologist can play a key role in treatment of the patient. Therefore, knowledge of its radiologic appearance and evolution is important in deciding the management.<\/p>\n<p style=\"font-weight: 400;\"><b><strong>RHINO-CEREBRAL MUCORMYCOSIS<\/strong><\/b><\/p>\n<p style=\"font-weight: 400;\">Paranasal sinuses are often the most common site initially involved in mucormycosis.\u00a0X-ray PNS findings:<\/p>\n<ul style=\"font-weight: 400;\">\n<li>Varying degree of opacification of sinuses.<\/li>\n<li>Rim of soft-tissue thickness along the paranasal sinuses.<\/li>\n<li>Complete sinus opacification, gas-fluid levels and obliteration of the nasopharyngeal tissue planes.<\/li>\n<\/ul>\n<h2 style=\"font-weight: 500;\"><b><strong>PULMONARY MUCORMYCOSIS<\/strong><\/b><\/h2>\n<h2 style=\"font-weight: 500;\">Intrapulmonary Imaging Manifestations: X-ray Chest Findings:<\/h2>\n<ul style=\"font-weight: 400;\">\n<li>Reverse Halo Sign<b><strong>:<\/strong><\/b> Ground-glass lesion with a peripheral rim of consolidation (almost always\u00a0<b><strong>pathognomonic<\/strong><\/b> for Mucormycosis)<\/li>\n<li>Peribronchial ground glass opacity: early sign<\/li>\n<li>Cavitation: early sign<\/li>\n<li>Lobar and segmental consolidation with absence of air bronchograms<\/li>\n<li>Unilateral infection: can quickly progress to contralateral lung if untreated<\/li>\n<li>Multifocal pneumonia pattern with bilateral consolidation: has a high correlation with increased mortality<\/li>\n<li>Multi-lobar distribution: has a high correlation with increased mortality<\/li>\n<li>Single or multiple nodules and masses are also common features<\/li>\n<li>Necrosis (because the fungus tends to invade the vasculature)<\/li>\n<li>Pleural thickening or effusion: May indicate spread in to pleural cavity<\/li>\n<li>Subcutaneous emphysema: Air in the intercostal space or subcutaneous soft tissues indicates chest wall spread.<\/li>\n<\/ul>\n<h2 style=\"font-weight: 500;\"><b><strong>DIAGNOSTIC CHALLENGES<\/strong><\/b><\/h2>\n<p style=\"font-weight: 400;\">One of the diagnostic challenges in patients suspected of having invasive fungal pneumonia is differentiating Invasive Pulmonary Aspergillosis (IPA) from Pulmonary Mucormycosis (PM).<\/p>\n<p style=\"font-weight: 400;\">Imaging findings that are more suggestive of PM than IPA include:<\/p>\n<ul style=\"font-weight: 400;\">\n<li>Presence of Reverse halo sign<\/li>\n<li>Presence of more than 10 lesions<\/li>\n<li>Presence of pleural effusion<\/li>\n<\/ul>\n<p style=\"font-weight: 400;\"><b><strong>FEW TYPICAL X RAY PATTERNS OF PULMONARY MUCORMYCOSIS (PM)<\/strong><\/b><\/p>\n<p><img decoding=\"async\" class=\"wp-image-2251 aligncenter\" src=\"https:\/\/suburban-blog.abymtech.com\/wp-content\/uploads\/2024\/12\/img-1-300x135.webp\" alt=\"\" width=\"638\" height=\"287\" title=\"\" srcset=\"https:\/\/www.suburbandiagnostics.com\/blog\/wp-content\/uploads\/2024\/12\/img-1-300x135.webp 300w, https:\/\/www.suburbandiagnostics.com\/blog\/wp-content\/uploads\/2024\/12\/img-1.webp 700w\" sizes=\"(max-width: 638px) 100vw, 638px\" \/><\/p>\n<p>PM appearing with the reverse halo sign in a 68-year-old woman. Frontal radiograph shows a large cavity with an air-fluid level in the right lower lung.
